The Psychological Impact of Acute and Chronic Illness is a well-researched and focused document giving a panoramic view of various clinical, social, psychological and inter-, as well as intra-personal effects experienced by persons who experience the transformation into patients.

The author’s emphasis on understanding each patient as a unique individual is a thread that runs throughout the book. This is highlighted by her quoting Sir William Osler, right at the beginning: “It is more important to know what kind of patient has the disease than what kind of disease the patient has.”

The author’s insights into disease-specific factors and patient variables, including premorbid psychological factors which determine the nature of response to becoming a patient are well presented with insights like, “in terms of personality functioning, a brain injury exacerbates what was there before.” Discussing a person’s relationship to his body under “the narcissistic injury of illness” she expounds on how people, who tend to fill their lives with work, sex or athleticism feel empty in response to illness in which the body is compromised and vice versa. Hence in healthy state it is important to use both mind and body to prevent such emptiness from setting in, and the professional care givers will do well to remember this while providing services to their patients.

Illness can be met with denial, and may lead to dependency; each of these can be adaptive or pathological. As medical professionals it is important that these major responses and adjustments are given due consideration in individual patients. Her thoughts about considering every serious illness as a “traumatic event” and providing the same type of psychological support to medically ill people was a new and a refreshing insight for me as a general practitioner.

The book also includes valuable discussion of psychological disorders like, depression, generalized anxiety disorder (GAD), panic disorder and post-traumatic stress disorder (PSTD) and their proper pharmacological treatments.

Overall I feel this book is worth a good read by all professionals and the family members who share any responsibility for looking after patients with long-term conditions.

When a tattoo tells a story - published in BMA News

At first glance Jean did not seem to be capable of challenging doctors' opinions. She was 84, likeable and had been through a lot of medical investigations and examinations.

Her case file, in which she carried all her medical notes, reports, and copies of her medical prescriptions, was very fat indeed and looked well used and rather tatty. The owner, in contrast, was very thin, frail and well-kempt, complete with good layers of make-up.

Jean was admitted for being breathless for the fourth time in a week.
'A smoker?'
Oh yes, virtually all of her adult life: cigarettes, roll-ups, cigars - even the odd puff on a hookah pipe while travelling. 'But I've lived life to the full,'she said. There was something unnatural about the calm of her countenance despite her gasping.

I rushed through the history taking and requested the nurses to apply an oxygen mask with salbutamol nebulisation. Seeing me position the stethoscope ear pieces, Jean politely parted her dress to expose her chest,almost as a well-practised reflex. As I looked at her exposed bosom, what I saw challenged a long-held opinion.

Examining tattooed bodies has always intrigued me in a slightly repulsive way. I do not like tattoos. They have always seemed a desecration of the sublime, like painting colour on to a flower, although I have to admit that the artistry of some of them is impressive.

My negative views of tattooing may have been influenced by my strict Christian upbringing and reinforced by medical school days highlighting the risks related to it and the spread of infections, particularly HIV and hepatitis B.

A host of other complications have been referred to in medical literature, some are simply allergies or reactions to the pigments used. But more serious complications such as granulomas, pruritis, photoallergic reactions and keloid formation s have been documented.

I've certainly seen plenty of them: lions, gods, mermaids and dragons blowing smoke. Then there are the words 'If you can read this you are too close..." was inscribed on the gluteal region of a young woman. One man I examined for chronic cough and excessive smoking had the entire poem, Footprints in the Sand, written on his anterior chest.

But, of all the pictures, designs and words I had seen on nearly every conceivable part of the human skin, I had never before seen the words that were tattooed on Jean's exposed and wrinkled breasts. They had been chosen carefully, in the hope that one day they would come to a doctor's aid: 'In case of arrest, do not resuscitate.'

I aspired to an A-grade British education, not a B from McDonald's

I was shocked to read that working in a McDonald's for two weeks will give students the equivalent of a GCSE B-grade exam pass (Mail).

I come from a Third World country where UK education and qualifications, at all levels, are held in very high esteem.

I worked hard for my chance to learn and consider myself fortunate to come to the UK for postgraduate studies. Getting academic qualifications from McDonald's has never been my idea of British Standards of education. I suppose there's no harm in getting a certification of completion of wok experience from a laundry, bank, surgery or other work-based organisation.

What is disturbing is making this the equivalent of an academic grade. It makes me wonder what trends will evolve from such a move? Will Starbucks, Mr Burger and Subway hand out academic equivalents in food science? What about aspiring students from other countries who want to come to the UK for college studies? Will they be able to bring their own McGCSEs based on work experience in their country of origin? Will working for two years be equal to a diploma? We're told school leavers think that the world owes them a living. Making more of them leave school with B-grade equivalent GCSEs will only increase the problem.

I agree with Professor Alan Smithers: accreditation of work experience as equivalent to a GCSE 'devalues the currency of public exams'.

My Inspirations - a nostalgic piece from student days

I love the mighty rivers that wind like giant silver snakes among the mountain-breasts on Mother Earth. Ever since my childhood these rivers have always been a thing of awe and wonder to me. The way they rush towards their destination to meet the chasm less ocean; the way they keep journeying in the noon-day heat and in the quiet calm of night, are eloquent sermons to my inner self.

Rivers cannot be halted; they either find a way or make a way. They must go on. Choosing the lowest places to travel upon, and dissolving (accepting) anything that comes their way, they go on. Rivers do not only change the land on which they flow, they also accept change. If they cannot go over a mountain, or make a way through it, they go around it but they keep on going, nourishing all forms of life that reaches them or is reached by them. Rivers are a source of inner peace to my troubled life.

As a boy, I often used to go to near- by river and sit on its banks, I always felt knitted close to the heart of God, and been praying. Now as I look back, I know my prayers had always been, “God, give me the virtues of a river…” Have you ever felt you couldn’t start a task? Of course, little and meagre as we are, we all have felt like that once in a while (often, perhaps, is more correct). As a child I remember an undefinable spiritual experience of watching the rising sun. The night was dark, so dark that I felt if I went out I would become even darker then I was, but I wanted to see how the night changed into a day. So I and dog Bravo, went to nearby hill and faced the east in a worshipful way. The night was dark and black, but the heaves were studded with tiny jewels of stars. Minutes passed by, perhaps, they were hours-to me they seemed as though they were ages. Nothing magnificent happened, but after a while, I noted the darkness had lessened, the milky whiteness had started spreading around us. Bravo barked a little, I cuddled him close to my body. The eastern horizon started changing hues slowly, very slowly, and too slowly for an impatient boy of ten years. But then the miracle took place, still slowly, but definitely the night retreated and the fiery ball of sun came up. I could now tell it was a beginning of a new day. There was no drama, no explosion, no noise, but the change was great. I must have stood there for a long time awe-stricken, worshipful with a reverence for small beginnings that are worth great changes. Well, as somebody said, “Sunsets are dramatic endings, but sunrises are inspirational beginnings and of the two, I like the later”.

My mother once told me a story of a mountain that cried because it could not walk. I felt sad for that mountain then. Since then I have seen many mountains, but one thing that always strikes me is the fact that mountains never cry. They only stand there bearing all the abuse of natural forces; they only stand firm, because they know their strength is in their great integrity. For this unwavering integrity, I love mountains. “God, I think you too are like a great mountain, enduring forever.” I wish I too would be like a mountain, for mountains do not cry. Walking down a city street one evening, I could not help but be attracted by tiny flickering lights displayed on the wayside dance-houses. The music was deafening even out on the road where I was. The lights glittered, twinkled, shone and attracted passers-by. And then suddenly, a phenomenal change took place within me. My memory transported me to the humble village of my beginning. Totally unknown to the outside world is a little village named Medar in the heart of Pakistan. Years back, I was born there. I played there with my childhood friends. Now standing in front of the luxurious house, I vividly saw in my mind’s eye the walls of my village bathed in the cool sheen of milky moonlight. The evening time was a shadowy period of my village. Tranquillity reigned there. There was no music except the music of silence. There was no light except the moonlight….

Now that I am old I thought, “What would I choose?” A moment later, my eyes were wet. Tiny beads of water rolled down my cheeks; even now I would prefer evening moonlight on mud walls to dazzling disco lights and deafening music.
